SCAN Palliative Care Model: A Clinical-First Approach by Homebase Medical

Homebase Medical, a division of SCAN Group, is pioneering a novel palliative care model designed to enhance support for Medicare Advantage’s most vulnerable members. This initiative emerges as SCAN Group, the organization behind SCAN Health Plan, finalizes its merger with CareOregon, a prominent Medicaid provider. The newly formed entity will operate under the name HealthRight Group.

Mirroring other major payers, SCAN Group actively manages a diverse portfolio of healthcare provider businesses. Homebase Medical, previously known as The Residential Group before its acquisition by SCAN in 2022, delivers crucial services including Medicare welcome visits, transitional care, and specialized palliative care for elderly individuals facing complex health challenges.

SCAN Group CEO Sachin Jain has expressed considerable enthusiasm for the substantial growth in home visits and the groundbreaking clinical innovations spearheaded by Homebase Medical. Central to these advancements is the development of a “clinical-first” palliative care model, poised to redefine industry standards.

“In observing current palliative care models, a prevalent trend appears to be what I term ‘labor arbitrage.’ This involves reassigning tasks traditionally performed by specialist palliative care physicians to general practitioners, or from generalist doctors to nurse practitioners, and so forth,” Jain explained in a statement to Hospice News. “Our approach at Homebase Medical is fundamentally different. We are committed to constructing care teams that possess a deep understanding of the specific diagnoses and symptoms each patient presents. This is truly a model that prioritizes clinical expertise above all else.”

To ensure the robustness and efficacy of this innovative model, SCAN has collaborated with some of the nation’s foremost palliative care experts, engaging them as consultants throughout the development process.

SCAN Group, a $4.3 billion non-profit Medicare Advantage organization, provides healthcare coverage to over 285,000 members across California, Arizona, Nevada, and Texas.

Homebase Medical is a key component of SCAN’s broader network of provider businesses. This network also includes Welcome Health, a geriatric home-based primary care group; Housecall Providers, specializing in in-home primary and palliative care; and MyPlace Health, a PACE program established as a joint venture between SCAN and Commonwealth Care Alliance (CCA). Furthermore, SCAN operates a dedicated mobile care team tailored to serve institutional special needs plan beneficiaries.

Demonstrating its commitment to comprehensive care, SCAN also runs Healthcare in Action, a medical group focused on serving individuals experiencing homelessness. This initiative currently deploys 11 mobile care teams across various communities in California, including Long Beach, West Hollywood, San Mateo, and San Diego, among others, ensuring healthcare access for underserved populations.
